Change of Address or Phone Numer
Please do not forget to tell the receptionists if you change your name, address or telephone number. We do need to know if you move out of the practice area
As providers of NHS care we are mindful of the NHS Constitution, which sets out the legal rights and entitlements of patients, in addition to the responsibilities that the public, patients and staff owe to one another to make the NHS operate fairly and effectively. The constitution can be found at:

The GPs are also bound by their professional code of ethics and are obliged to fulfil the duties and responsibilities of a doctor as set out by the GMC. These are summarised under ‘good medical practice’ and available on:

Hearing Aid Batteries
Patients needing batteries for hearing aids should ask at the Community Health Team Reception on the 1st floor of the Health Centre.
Help the Practice to help you
Please do:
- Arrive on time for your appointment
- Inform the practice if you cannot attend - your appointment can be given to someone else
- Write down a short list of problems or questions if you are nervous or cannot remember
- If you do not understand what you have been told, say so to avoid confusion later
Please do not:
- Bring too many problems at one time – it is better to ask for more than one appointment
- Use one appointment for more than one patient
- Request a home visit unless it is for someone who is too ill or infirm to come to the surgery
